To apply for forgiveness of your Paycheck Protection Program (PPP) Loan, you must complete the attached application and submit to the Lender that is servicing your loan.
The attached application contains the following:
1.) PPP Loan Forgiveness Calculation Form
2.) PPP Schedule A
3.) PPP Schedule A Worksheet
4.) PPP Borrower Demographic Information Form (optional)
* All borrowers must submit items 1 and 2 to their Lender.
** Borrowers can complete this application electronically through their Lender.
